Physical Forms

These physical forms must be completed by the time camp starts on August 11th for any athlete to be able to participate. The CIPPE Form, which you can download through the link below, is now the only form accepted by the PIAA. The school offers physicals after school - check with Coach Read (gread@dasd.org) for the dates.

Also, don't forget about the $10 activity fee required of all athletes. It is a fee paid only once during the year, no matter how many sports you play. Please get it in to Coach Read or Coach McCray at the beginning of the season!
